People search how long is a rail medical valid because diary planning and Sentinel status both depend on the answer. Absolute Biomedical publishes a clear age-linked rule. This article restates it carefully, explains what can shorten practical fitness even when the certificate date looks fine, and shows how medical validity differs from drug and alcohol re-test rules.
Absolute Biomedical’s published validity bands
As a general rule, medical validity is linked to your age at the time you attend and pass a medical. The maximum expiry date of medical certificates issued shall be:
- 10 years, up to the age of 40
- 5 years, until the age of 65
- Renewed annually thereafter
So a candidate who passes at 35 may receive a longer maximum validity than someone who passes at 55, and someone over 65 is on an annual renewal cycle. Always read the certificate/upload you actually receive — the bands describe maximums under the published rule.
Illness, injury and change of circumstances
Absolute Biomedical’s FAQ adds an important caveat: if at any time an individual is subject to significant illness, injury or change in personal circumstances that could impact their fitness to work, they should discuss this with their primary sponsor, who may refer the individual to an occupational health assessment before normal work is continued.
In other words, the printed expiry date is not a guarantee you remain fit until that date no matter what happens. Sponsors have a duty to manage fitness dynamically.
Medical clock vs D&A clock
| Trigger | Medical | Drug & alcohol |
| New entrant | Level 1 medical required | New D&A required |
| Age / time | Expiry by age band above | Not the same as medical years |
| Change primary sponsor | Only if medical expired / sponsor requires | New D&A if last screen >12 weeks |
| Random / for-cause / incident | Not automatic | Employer-arranged screens |
Confusing these clocks is a common booking error. A medically valid worker can still need a fresh D&A for sponsor change; a recently screened worker can still hold an expired medical.
Planning renewals before you hit the wall
- Diarise expiry at least several weeks ahead — allow appointment lead time plus any referral.
- Medical results often upload within 24 hours unless referral; referrals add unknown time.
- If you also need D&A for other reasons, consider combining visits (Post 10).
- Over-65 annual renewals need a standing reminder in sponsor systems, not memory.
What the medical itself includes (reminder)
When you renew, expect the same broad Level 1 structure Absolute Biomedical lists: questionnaire, blood pressure, BMI, mobility, audiometry, vision and colour vision, general health — typically 40–60 minutes with an approved examiner against NR/L2/OHS/00124 expectations for track workers.
